We do not accept prescription requests over the phone for reasons of safety and accuracy.
All repeat requests should be submitted either:
- via the NHS Wales app
- via your chosen pharmacy
- by bringing the completed repeat prescriptions request slip and dropping it into the practice.
- or by emailing us at admin.w91016@wales.nhs.uk
If you have lost your prescription request slip please request your medication in writing supplying your name and your date of birth.
Please allow us up to 72 hours for processing your request.
We are only able to accept pathology samples from patients, which have been requested following consultation with a GP or Practice Nurse. We will no longer accept samples from patients for routine testing or checking.
If the GP/Practice Nurse gives you any forms during your consultation relating to the urine sample, please ensure you return this to the surgery with your sample. This will help us to process your sample.
If you feel you may have a water infection, you can now receive treatment free of charge from your local Independently Prescribing Pharmacist. Alternatively, you can arrange a telephone consultation with a clinician or complete an AccuRx Triage. via https://florey.accurx.com/p/W91016 giving as much information as possible regarding your symptoms.

The practice has the ability to use text messaging to communicate with our patients. We use this method in a variety of ways which include appointment reminders and contact regarding text results. If you wish to opt out or decline to be contacted by text messaging please inform our reception team.
