New Patients
How to register
To register as a patient, please check that you live within the practice boundaries, this information is available at Reception.
You will need to complete an NHS Registration Form and a new patient questionnaire (both can be downloaded below or collected from reception)

Please ensure you complete the form fully and sign the registration form at the bottom of the first page and return the forms to us either via reception or by emailing them to: Admin.W91016@wales.nhs.uk
We will then apply for your full health records, this can take some weeks so please do not be alarmed if we do not receive your health records straight away. Patients can still see our clinicians during this time.
